New Patient Registration

To register please ask at reception. You will be given a form and new patient questionnaire. It would be helpful if you have details of your previous doctor’s name and address. This will enable the Practice to get your medical notes from your last GP.

Please note we are not a dispensing practice.

Please print a Patient Questionnaire and either email it back to us @ newburystreet.admin@nhs.net or drop it into the surgery.

Newbury Street Questionnaire and Online Access Application

If you have a complex medical history or are on medication it would be helpful if you arranged a routine appointment with your named GP so they can get to know you.

Medical treatment is available from the date of registration.

Below is a useful practice guide to see how we work at Newbury Street Practice and how to go about requesting medication and accessing appointments both now and in the future
